PASSPORT SERVICES OFFERED AT THE LIBRARY—BY APPOINTMENT ONLY
The items needed to apply for a passport are:
Completed application IN BLACK INK- unsigned. (can be done online at www.travel.state.gov); Must be 2 separate pages NOT DOUBLE-SIDED. Applications can be downloaded from www.travel.state.gov, and filled it out at home, except for the signature. We also have applications that can be picked up at the library.
CERTIFIED BIRTH CERTIFICATE WITH A RAISED SEAL from the state in which you were born OR A US PASSPORT. (To obtain a certified birth certificate from Pennsylvania, go to www.dsf.health.state.pa.us, or phone 724-656-3100.)
Two passport photos with white backgrounds (from Sears, AAA, Picture People, Bower’s Photo, or WalMart; we do not take passport photos)
Valid PA photo driver's license OR, if out of state, 2nd ID required
2 Checks (SSPL - $25.00); (US Dept. of State - $110.00 adult; $80.00 child under 16. For a PASSPORT CARD: $30 adult, $15 child)
Expedite fee: $60.00/each application
Minors under 16 years old MUST have both parents present or a notarized form from the absent parent!!
Passports are by appointment ONLY:
Monday: 5:00 – 7:30 PM
Tuesday: 4:00 – 5:45 PM
Wednesday: 4:00 – 7:30 PM
Thursday: 4:00 – 7:30 PM
Turnaround:
4-6 weeks - Standard
2-3 weeks - Expedited
